VNA Northwest based in Bantam is seeking the public's help in identifying senior citizens who may need help in connecting to services to allow them to continue living safely in their homes.
With grant funding from the East Hill Woods Fund of the Connecticut Community Foundation, VNA Northwest is equipped to identify and meet with up to 30 residents of nine western Connecticut towns, with the goal of connecting them to services such as the state's Connecticut's programs for Health insurance assistance, Outreach, Information and referral, Counseling and Eligibility Screening program.
The towns included in the grant's guidelines are New Milford, Washington, Warren, Bethlehem, Goshen, Litchfield, Morris, Thomaston and Woodbury.
VNA Northwest's staff social worker, Mindy Garbin, MSW, recently completed a training session about CHOICES and other services available for local seniors through the Western Connecticut Area Agency on Aging.
She and VNA Northwest staff nurses are prepared to share the information with seniors who may not be aware of these options.
